Consider the following scenario:
You are the paramedic working with an EMT partner who has about 8 months of experience. It’s 4:17AM and you are on a 24hr shift. You have been asleep for about 45 min when you are called to a local apartment complex for a 1 month old male having a cardiac emergency.
On arrival you find a 1 month old (4kg) infant being held by his mother. The parents advise that the child has been diagnosed with an atypical SVT and is scheduled for definitive treatment next week. The infant is pink and appears irritable, airway is maintained, and his RR is approximately 37/min. VS are as follows: HR: 236, regular, narrow complex | BP: 88/46 | SaO2: 98% on room air. He is afebrile.
You decide to administer adenosine. You have 6mg/2ml on hand. How much will you give?
If you are using your OneDose app you simply find Adenosine, enter the weight, and BOOM….you get how much to give. There is also a shortcut way to do a quick 1:1 dilution with saline, use a three-way stopcock, and pull up 0.4mg ….but that still requires some time to setup and you have to know how. Remember, it’s 4 in the morning and you are fatigued with no paramedic partner to “bounce things off of.” The last way is to do the math that we all learned in medic school and come up with 0.13mg. That’s fine too, but the reality is that many people can’t do that calculation on a whim – especially when fatigue and nerves are factored in.
Here's another one:
Right after you clear the call above – around 5:45AM (which was well managed since your agency did have the OneDose app) you are called to an allergic reaction. You find a 35 y/o F who accidentally ate a breakfast bar containing peanuts. She has a severe nut allergy and presents with acute SOB, hives, and hypotension. She is talking in broken sentences. Her husband has administered an Epi-Pen and also knew how to take it apart to get additional doses. He administered a second dose 3 min before your arrival (total time in is about 7 min.) You ask your EMT partner to pull up an additional dose of Epi and start an Albuterol neb while you obtain access. She is getting no better and you decide to administer 100 mcg of PDP Epi while getting an Epi drip setup. Your protocols for mixing are 8mg Epi in 250ml NS. You are to start the drip at 0.1mcg/kg/min. As the patient sits on your cot you tell your EMT partner to pull up an Epi drip on the OneDose app. Fortunately, your agency also recently installed the OneWeight cot scales onto all units and you note that her weight is 59kg on the display. This is already transferred to the OneDose app and your partner shows you the screen with the appropriate drip rate to set.
Now, I know what some of you are saying – just do a “dirty Epi drip” till she gets better. That’s fine, but this agency has an established protocol for an Epi drip and the Medical Director expects that protocol to be followed. I can also hear the folks in the back of the room saying “why are you calculating anything….just use a pump!” Again, a fine solution IF YOU HAVE IT. Let’s be honest, many Fire/EMS agencies are not carrying pumps on 911 trucks. There is also the possibility that the pump malfunctions or the battery is dead – then what? It’s akin to pilots saying, “why are you landing the plane, just let the Autoland system do it.” If that system fails, we all hope the pilots can fall back on their training and land the plane.
